I live in Italy and I'm really happy about strength of SpartacusFilter( www.spartacusfilter.com ). It's free for private use.
It's a light plug-in for Outlook express, Microsoft Outlook, Exchange server, IBM Lots Notes or IBM Domino Server.
It's a self-learning tool, artificial-intelligence(!) based.
It's wrong only a very few times and it's able to change his behaviour.
After some training it's detection rate is near 99%.
Don't ask to me to say more... I prefer you give to it a try! -

I have used Mailwasher for years now and am well satisfied. The bought version copes with multiple email accounts and even has a recover facility if you realise a valid email got dumped. Blacklists, whitelists and links to subscriber filters are catered for. I cant remember how much i paid but it was less than a hundred $. mailwasher downloads headers for semi automatic review, and will bin the emails off your server(s) before you hit send/rec in Outlook
